In March 1975, biological warfare between the People's Republic of China and the Soviet Union kills most of the world's population. U.S. Army Lieutenant Colonel Robert Neville, M.D. (Charlton Heston), a scientist based in Los Angeles, California, begins to succumb to the plague but injects himself with an experimental vaccine just in time, rendering himself immune. The plague's surviving victims, meanwhile, join together as "The Family," a cult of nocturnal albino mutants who seek to destroy all technology.
Two years later, Neville believes he is the plague's only survivor, struggling to maintain his sanity. He spends his days hunting and destroying members of the Family. At night, living atop a fortified apartment building equipped with an arsenal of automatic firearms (such as a Smith & Wesson M76 submachine gun and an infrared sight-equipped Browning Automatic Rifle), and explosives (such as satchel charges), he is a prisoner in his own home. The Family wants to destroy him, believing him to be a last remnant of the old culture.
One day, the Family captures Neville in a wine cellar. After a summary trial he is found guilty of heresy by Jonathan Matthias (Anthony Zerbe), a former news anchor who is now leader of the Family. Neville is sentenced to death and nearly burned at the stake in the center of Dodger Stadium. He is rescued by Lisa (Rosalind Cash), a woman he had earlier seen while on patrol, and Dutch (Paul Koslo), a former medical student familiar with Neville's work.
Lisa and Dutch are part of a group of unmutated survivors; although infected, their youth has given them some resistance to the disease and its symptoms are slow to manifest. Nevertheless, given enough time, they will succumb to mutation and become new members in the Family. Neville is amazed to find that some of the survivors include very young children. Neville later has a brief romance with Lisa.
Neville realizes that even if it is possible to duplicate the original vaccine, it would take years to salvage humanity. However, he believes it may be possible to extend his immunity to others by creating a serum from his blood. If the serum works, Neville and Lisa plan to leave the ravaged city with the rest of the survivors and start a new life in the wilderness, leaving the Family behind to die.
Neville is successful in creating the serum and administers it to Lisa's teenage brother Richie (Eric Laneuville), who is on the verge of the advanced mutant stage of the plague. Once cured, Richie (insisting that members of the Family are also human) goes to the Family to try to convince them to take the serum. Matthias refuses to believe that Neville would try to help them, accuses Richie of being sent by Neville to harm them and has Richie executed. Neville discovers Richie’s body strung up and left as bait to draw Neville outside after dark. Enraged, Neville fights off the Family after they force his car off the road.
Meanwhile, Lisa unexpectedly changes into a nocturnal albino mutant
and betrays Neville by giving the Family access to his bunker. Returning
home, Neville is confronted by Matthias, who forces him to watch as the
Family sets his home on fire. Neville breaks free and, once outside
with Lisa, he turns and raises his sub-machine gun to shoot Matthias,
who is looking down from the balcony. The gun jams, giving Matthias
enough time to hurl a spear at Neville, mortally wounding him. It is
assumed that Matthias and the Family choose to stay inside Neville's
home as it continues to burn. Lisa claims to be part of the Family, but
stays by the fountain with Neville until dawn.The final scene shows the human survivors, led by Dutch, departing in a Land Rover. They discover a dying Neville, who hands Dutch a flask of blood serum, presumably with which to restore humanity. Shortly after handing over the serum Neville dies in a fountain, Dutch finds the spear and briefly looks over it before throwing it away, Neville symbolically posed in the traditional position of the crucified Christ. Dutch takes Lisa and the survivors away as they leave the city for good, leaving the Family to die, as Neville planned.

In una Los Angeles abbandonata e spettrale si aggira Robert Neville, ex medico militare e ultimo uomo sopravvissuto ad un olocausto batteriologico avvenuto due anni prima. La ricerca di generi di prima necessità da una parte e quella di mantenere la propria mente lucida in una realtà alienante dall'altra ne caratterizza le giornate, mentre la lotta armata per la sopravvivenza ne caratterizza le notti. La solitaria esistenza di Neville è infatti messa a repentaglio dal tramonto all'alba dagli assalti della Famiglia, una setta di creature che i veleni della guerra batteriologica hanno trasformato in fotofobici e psicotici albini. Riuniti attorno alla farneticante figura del loro capo Matthias (Anthony Zerbe), questo manipolo di poche centinaia di individui abbandona il proprio rifugio - simbolicamente il municipio della città - e esce allo scoperto non appena il sole cala dietro l'orizzonte per distruggere con il fuoco qualunque retaggio dell'era tecnologica, responsabile della distruzione della civiltà e di cui Neville rappresenta chiaramente l'ultima, detestabile incarnazione.
Inesorabilmente scandita da luce e tenebre, la monotona successione di giornate sempre uguali si arresta quando Neville scopre di non essere realmente solo. L'incontro con Lisa, Dutch e un gruppo di bambini miracolosamente scampati ad una seria intossicazione, ma di cui presentano comunque sintomi leggeri, fornisce a Neville un nuova ragion d'essere. Il suo sangue, immunizzato da un vaccino sperimentale di sua ideazione, rappresenta la salvezza per un'umanità senza più speranze. Nonostante sia riuscito a salvare Richard, un ragazzino al secondo stadio avanzato della malattia, Neville è contrario all'idea di curare la Famiglia. Programmata la partenza per abbandonare la città e trasferirsi in montagna insieme agli altri giovani superstiti, Richard si presenta al municipio per convincere Matthias e i suoi esponenti a farsi curare. Questi però dubitano delle parole del ragazzo e lo uccidono. Precipitatosi al loro nascondiglio, Robert Neville non trova nessuno; nel frattempo Lisa, ormai allo stadio finale della malattia, si imbatte nella Famiglia da cui viene inevitabilmente attirata. L'ultimo incontro tra Matthias e Neville avviene nell'attico di quest'ultimo, che riesce a trarsi in salvo dall'aggressione della Famiglia e portare con sé anche Lisa nella speranza di poterla curare, ma Matthias coglie l'attimo e scaglia una lancia a Neville, colpito proprio davanti casa sua. All'alba arriva il gruppo di Dutch che trova Neville morente e Lisa in pessime condizioni; prima di esalare l'ultimo respiro Neville consegna nelle mani di Dutch il flacone contenente il siero per salvare l'umanità. Nell'ultima inquadratura Neville appare senza vita nella fontana davanti casa sua con le braccia allargate, nella stessa posizione di Gesù crocifisso, il sangue sgorgante dalla ferita si è ormai mischiato all'acqua. Neville è il nuovo salvatore dell'umanità.

como parte de los ataques bacteriológicos se ha liberado un virus que
acaba siendo mortal, y que ha exterminado la población mundial. Robert
Neville (Charlton Heston), un científico militar de la ciudad de Los Ángeles,
había preparado a tiempo una vacuna que se inyectó y que le salvó de la
muerte. Han sobrevivido también unos centenares de individuos que se
llaman a si mismos La familia, liderados por Jonathan Matthias (Anthony Zerbe),
a los que los agentes biológicos les han causado una hipersensibilidad a
la luz, por lo que se esconden durante el día en el subsuelo de la
ciudad, y salen a la superficie durante la noche. Sienten la necesidad
psicótica de acabar con todo lo que tenga que ver con la ciencia y la
tecnología, las que consideran causantes de la guerra. Por ello, cada
noche intentan entrar en el lugar donde vive Neville para matarle, pero
éste se atrinchera con todos los medios a su alcance.Un día Neville es capturado en una bodega de vinos, y es sometido a un juicio dirigido por Jonathan Matthias, que lo condena a morir en la hoguera. Dos supervivientes aún no infectados por el virus, Lisa (Rosalind Cash) y Dutch (Paul Koslo) logran rescatarlo. Ella lo había visto cuando recorría la ciudad, y él es un estudiante de medicina, familiarizado con las investigaciones de Neville. Ambos son parte de un pequeño grupo que ha sobrevivido al virus y a las matanzas "purificadoras" de la familia, en su mayoría jóvenes y niños que tampoco habían sido afectados por el virus. Con ello, nace una nueva esperanza para los supervivientes, que abandonarán la ciudad para comenzar una nueva vida en algún lugar, pero antes volverán a enfrentarse con La Familia, siendo atacados por sorpresa, lo que va a desencadenar un combate que los obliga a dejar a Neville, herido de muerte, tras ellos.

À Los Angeles en 1977, Robert Neville, un ex-médecin militaire, est l'un des rares survivants d'une guerre biologique entre l'Union soviétique et la Chine ayant tué la plupart de la population humaine en raison d'une épidémie mondiale qui a suivi, mais contre laquelle il était immunisé grâce à un vaccin expérimental, le 93-B71, qu'il avait fabriqué et s'était lui-même injecté à titre expérimental.
Subsistent cependant d'étranges êtres déformés qui se nomment eux-mêmes "La Famille" et dont les séquelles de la guerre en ont fait des mutants albinos nocturnes et très sensibles à la lumière. Menés par un chef psychotique, Jonathan Matthias, ils pensent que la science et la technologie sont la source de tous leurs malheurs et doivent être détruites. Vêtus de robes noires, ils n'utilisent que des objets primitifs (torches, arcs, flèches, catapultes...) et tentent d'effacer toute trace de progrès technique. C'est ainsi qu'ils considèrent Neville comme un vestige de cette société honnie et cherchent à le tuer.
À l'inverse, Neville, utilisant toutes les ressources technologiques disponibles, a aménagé un appartement situé au sommet d'un immeuble qu'il a fortifié et tient La Famille en respect grâce à un arsenal d'armes modernes (pistolets et fusils mitrailleurs, charges explosives, etc...).
Il ne sort que le jour afin de s'approvisionner dans les magasins abandonnés de la ville tout en cherchant le repaire de La Famille. La nuit tombée, il illumine son immeuble et les alentours afin de décourager les mutants qui se rassemblent néanmoins tous les soirs sous ses fenêtres en brûlant des livres et autres objets tout en vociférant.
Neville finit toutefois par être capturé et, après un simulacre de procès, est déclaré coupable d'hérésie et condamné à être brûlé. Il échappe cependant de peu à sa sentence grâce à une femme qu'il avait aperçue un peu plus tôt lors de l'une de ses patrouilles. Il découvre alors qu'il existe un autre groupe de survivants, non immunisés contre l'épidémie, mais qui n'ont pas mutés et dont les enfants ont développé une résistance partielle à la maladie, leur permettant de rester à un stade pré-albinos. Le temps joue néanmoins contre eux et ils sont destinés à succomber à terme à l'épidémie, et à ainsi devenir de nouveaux membres de La Famille.
À la demande de ses nouveaux partenaires, Neville accepte d'essayer de recréer le vaccin 93-B71, mais il réalise que même s'il y parvenait, il lui faudrait des années pour sauver l'humanité. En revanche, la solution serait peut-être d'étendre son immunité aux autres en créant un sérum à partir de son sang. Une fois fabriqué, le sérum est injecté à Richie, le petit frère de Lisa, qui est sur le point de devenir lui-même un mutant.
L'expérience réussit et, une fois guéri, Richie part à la rencontre de La Famille pour essayer de les convaincre de prendre le sérum. Mais Matthias refuse de le croire, l'accuse d'avoir été envoyé par Neville pour leur nuire, et le tue.
Pendant que Neville part à la recherche de Richie (dont il finit par découvrir le cadavre), Lisa devient elle-même une mutante et le trahit en permettant aux membres de La Famille d'accéder à son immeuble fortifié. À son retour, Neville doit faire face à Matthias qui le contraint à assister à la destruction de son repaire. Il parvient toutefois à se libérer et, une fois dehors, tire au pistolet-mitrailleur sur Matthias qui profite de l'enrayement de l'arme pour lui lancer un javelot qui le blesse mortellement.
Le film se termine sur le départ à bord d'un véhicule d'un certain nombre de survivants munis du sérum que Neville, juste avant de mourir, leur a confié.
